RESPONSIBILITIES AND DUTIES INCLUDE, BUT ARE NOT LIMITED TO: • Reports directly to the Community Support Supervisor(s) or other person as assigned; • Promote and maintain the vision, mission and policies of Community Connections Inc. and the people it supports; • Responsible for maintaining and promoting the wellbeing, uniqueness and independence of each client by following present case plan; • Provide leadership and support clients by promoting the integration and acceptance of clients in the community; • Must be able to multi-task and work in a high-stress, fast-paced environment; • Responsible for the development and implementation of client case plans. Monitor and evaluate client case plans and attending meetings as required; • Responsible for engaging and supporting clients with their development which may include individualized home-based day services; • Fulfill the health, safety and personal care requirements for each client; • Assist clients with all aspects of daily living, including but not limited to personal care, arranging appointments, transportation, accessing the community and financial support, as needed; • Responsible and maintain all relevant documentation; • Ability to work collaboratively with Day Services/Residential employees; • Provide critical thinking and decision-making skills; • Ability to work independently and under minimal supervision; • Effective oral and written communication skills; • Other duties as assigned.
QUALIFICATIONS: • Minimum of a College diploma in Human Services and/or University degree in the Human Services field; • Previous experience working with individuals with cognitive and/or physical challenges or previous experience in a human service setting serving vulnerable populations would be considered an asset; • Current and valid CPR and First Aid certificates (must be maintained during employment); • Current certification in Mandt (must be maintained during employment); (will provide training) • Must possess and maintain a valid driver’s license and use of personal vehicle during working hours; • Computer skills; • Team player with excellent attendance; • Must provide a clear Criminal Record Check – Vulnerable and a clear Driver’s Abstract.
Applicants must possess a good safety record on a continual basis. Selection of employees for appointment is based upon such factors as ability, special training, experience, job performance record, attendance, education, and length of service, in accordance with the Collective Agreement.
*Subject to successful completion of probationary period as per Article 9.04 of the Collective Agreement.
